Miles for Smiles Visit to Everton School

February 18, 2010, Miles for Smiles returns to Everton! If you were seen last year by the Miles for Smiles team, we want to be sure to see you back this year. If we did not see you last year, it is time to get your teeth cleaned and examined.
Please return the consent form that the school nurse sent to you. Complete the front, back and BE SURE TO SIGN THE BACK OF THE CONSENT.
The program is available to students who 1)have Medicaid, 2)most MC+ or Medicaid Managed Care Plans, or 3) are uninsured. If uninsured, you will be required to pay a private fee, in full, the day services are rendered.
